Provide terminally ill patients with the necessary physical, emotional and spiritual support to ensure the best quality of their remaining lives, allowing them to die with dignity; and to provide patients' families and friends with emotional support for the patients' lives and thereafter.

 
For over 20 years...

Not all hospice care is the same. People make the difference. Staff, volunteers, patients, families, donors, board members and many more have all helped to create the vital community asset that is Sangre de Cristo Hospice & Palliative Care.

Sangre de Cristo Hospice & Palliative Care has been providing comprehensive, compassionate end-of-life care to our communities. Whether at home, in a skilled nursing facility, or our inpatient facility, our teams of nurses, doctors, CNAs, chaplains, counselors, and volunteers help ensure that patients and their loved ones have total physical, emotional, and spiritual support through perhaps the most difficult time of their lives. Through pain management, symptom control, emotional support and spiritual guidance, we place the emphasis on living life to its fullest up to the final moment.

We are grateful for the trust that you have placed in us to see that your loved ones, parents, family, friends, neighbors, co-workers and many others need not die alone or in pain. In 2007, more than 800 patients and their loved ones in southern Colorado experienced the excellence of care we provide during this significant time.

We remain committed to making sure that no one ever dies alone or in pain and are honored by the opportunity to serve the individuals and families of Southern Colorado.

 

 
What can you expect from Hospice?

Your hospice benefit provides the following hospice services for the terminal illness and related conditions:

 Expert Nursing Care & Symptom Management
 Social Worker Services
 Skilled Physical Care by Certified
      Nursing Assistants
 Patient & Family Teaching
 Spiritual Support
 Volunteer Support
 Grief Counseling for Patient & Family
 Medical direction and advice from hospice
      physicians

Sangre de Cristo Hospice & Palliative Care is available 24 hours a day to include weekends and holidays. Care from a provider not authorized by the elected hospice may not be covered.

Patients are accepted on the basis of medical need. Benefits and services are provided without regard to age, race, color, sex, sexual orientation, national origin, religion, or ability to pay.
